Director of Patient Services (Full Time)

Director of Patient Services needed for a medium sized licensed home care agency. We are looking for a Registered Nurse with community connections, proven leadership skills, and experience with growing an existing operation in the Western New York and New York City regions.

The Director of Patient Services is responsible for maintain patient care in accordance with Article 36 Licensed Home Care Agency regulations. Responsibilities include:

Responsible for direction, oversight, supervision, and management of, day to day operations within the nursing department of the LHCSA and NYS waiver programs within the agency including but not limited to PCA’s, HHA’s, RN field Nurse Supervisors and Staff RN/LPN’s. Ensures that all aspects of home care, programs, and staff operate within and maintain the standards of all applicable laws, rules and regulations according to best practices and NYSDOH.
Utilizes effective problem solving skills and decision making approaches to maximize effectiveness of the department and the care provided.
Management and implementation of incoming referrals for new cases, and coordination of care including but not limited to collaboration with discharge planners/social workers in LTC facilities/ hospitals/ MD offices/Insurance referral sources, including evaluation for appropriateness for home care services and staffing availability.
Collaboration with Clinical Administrator and clinical team regarding incoming waiver referrals, and current waiver participants for coordination of care and appropriateness for programs/ needs/staffing etc.
Development, management, maintenance and revision of plans of care, detailed service plans, individual service reports, Serious Reportable/Recordable incidents, MD orders, CQI reports as needed.
Attend Clinical Team Meetings at patients’ homes or other designated areas as needed.
Travel between NYC and WNY
Ensure ongoing training to meet NYS annual requirements for all staff, as well as participation in orientation and annual trainings as needed.
Respond to and manage complaints/concerns from clients, families and outside providers through to resolution.
Maintain professional networks and good working relationships with outside agencies/ providers. Maintains professional boundaries and models positive behavior and communication.
Oversees employee disciplinary process, annual evaluations et al. Collaborates with HR and carries out results of each as needed/indicated, providing retraining, education and guidance as needed.
Participation in various committees/meetings including but not limited to Incident Review Committee, Nursing Dashboard, Referral Pipeline, Supervisors meeting etc.
Ongoing business development.
Development, review and revision of all policies and procedures in accordance with the needs and rights of the clients and caregivers in accordance with QA/Corporate Compliance recommendations, and governing NYS DOH rules and regulations.
